The LVN plays a critical role in delivering compassionate care in a structured and supportive environment.
- Administer prescribed medications and perform treatments, including wound care, tube feeding, and other specialized nursing interventions, as required.
- Provide patient education on medications, treatments, and lifestyle recommendations to promote better health outcomes and independence.

Job Description
The Licensed Vocational Nurse (LVN) performs advanced vocational nursing duties to ensure the health, safety, and overall well-being of individuals with intellectual disabilities, complex medical needs, mental health conditions, and/or behavioral challenges. The LVN plays a critical role in delivering compassionate care in a structured and supportive environment.
- Administer prescribed medications and perform treatments, including wound care, tube feeding, and other specialized nursing interventions, as required.
- Provide patient education on medications, treatments, and lifestyle recommendations to promote better health outcomes and independence.
- Ensure complete and accurate documentation of patient care, assessments, and treatments in compliance with facility policies and healthcare standards.
- Utilize computer systems for data entry and care documentation (training provided to ensure proficiency).
- Collaborate with interdisciplinary teams, including physicians, therapists, and direct support staff, to develop and implement individualized care plans.
- Respond promptly to medical emergencies, providing appropriate interventions and notifying the healthcare team as needed.
- Monitor patient health and behavior, identifying changes or potential concerns, and reporting them to appropriate personnel.
- Ensure compliance with facility Standard Operating Procedures, including ICF-IDD Regulations, facility policies, and standards, rules, and directives.
- Participate in staff meetings, training, and continuing education to stay informed of best practices and maintain licensure.
- Maintain a safe and clean environment for individuals by following infection control protocols and ensuring proper use of medical equipment
