This health system provides access to primary, specialty, tertiary, and urgent care, all within one complete health system. Our client is a Level I trauma center, and is consistently recognized for its quality, safety, and exceptional patient care.

About the Role:

Nucleus Healthcare is seeking a dedicated Home Health Registered Nurse (RN) to provide high-quality, in-home care to elderly patients, helping them maintain their independence and quality of life. As part of an interdisciplinary care team, you will develop personalized treatment plans, deliver skilled nursing services, and support patients in managing their health at home.
Key Responsibilities:
- Conduct in-home assessments and develop individualized care plans.
- Provide education to patients and caregivers on disease management and treatment plans.
- Coordinate care with physicians, therapists, and other healthcare professionals.
- Ensure accurate and timely documentation in compliance with regulatory guidelines.
- Monitor patient progress and adjust care plans as needed.
Minimum Requirements:
- Active Registered Nurse (RN) license in [State].
- AHA BLS Certification (required).
- Minimum 2 years of home health nursing experience (required).
- Valid driver’s license and reliable transportation (required).
