Nucleus Healthcare is seeking a Certified Home Health Aide to join our trusted partners' prestigious home care facility in Gloucester County, New Jersey.
Job Title: Certified Homemaker - Home Health Aide
Shift Information: FT; 8:00AM - 8:00PM; 3x12 Hour Shifts
Location: Gloucester County, New Jersey
Job Description:
The Home Health Aide provides personal care and related services in the home. The aide functions under the direction, instruction and supervision of the staff nurse and the Director of Nursing and/or appropriate supervisor.
Responsibilities:
- Assist with ADLS and ADLs as per the POC
- Follow the POC for each client and document the same
- Help client maintain good personal hygiene
- Plan and prepare nutritious meals. Markets when indicated on the POC
- Assist the client with ambulation as per the POC
- Assists the therapy personnel as needed with rehabilitative processes
- May cue for medications ordinarily self-administered as assigned
- Taking and charting vital signs as per the care plan
- Assist with assigned treatments as ordered by the physician and approved and supervised by the nurse
- Prepares a report of visit on the day it is performed and incorporates same in the clinical record weekly or as directed
- Reports any change in the client’s status or home situation to her immediate supervisor, the staff nurse, or the aide supervisor
- Carries out their assignment as instructed by the nurse or the paramedical team and documents and reports to the nurse when they're unable to do so
- Performs routine housekeeping tasks as related to a safe and comfortable environment for the client, as instructed by the professional nurse
- Attends in-service as required by state and federal regulations
- Confirming weekly, the scheduling of visits with the Supervisor/Director, to coordinate necessary visits with other personnel.
